To Request Prescriptions or Records:

All Prescription or records requests must be made in one of the following two ways:

1. Request the records in-person by stopping by our office during normal business hours.

2. Call the office at 775-787-3939 to request us to fax you a records release form. Fax the completed form back to the office and we will fax your records to you
